I agree to abide by all of the policies and procedures governing TTU K-12. (The Policies are available online at k12.ttu.edu, or obtain a printed copy by calling 800.692.6877.) TTU K-12 policy and applicable law governing the release of student education records permits TTU K-12 to release grades for students under 18 years of age to the students' parents or legal guardians without requiring student permission. If the custodial parent does not want TTU K-12 to release grades or other educational information to the non-custodial parent, a court or other legal decree that clearly states that the non-custodial parent does not have any educational rights with regard to the child must be provided to TTU K-12. However, a student 18 years of age or older must grant permission for TTU K-12 to release grades to parents or legal guardians. In addition, TTU K-12 policy and applicable law permits TTU K-12, without prior consent and regardless of age, to release a student’s grades to a requesting school if the student is enrolled in or transferring to the requesting school. I certify that this information is complete and correct to the best of my knowledge. I authorize the University to verify the information I have provided. If applicable, I also authorize TTU K-12 to electronically access my Texas Success Initiative (TSI) information. Family Educational Rights and Privacy Act (FERPA)/Directory Information: The Family Educational Rights and Privacy Act (FERPA), a Federal Law, requires that Texas Tech University K-12 (TTU K-12), with certain exceptions, obtain your written consent prior to the disclosure of personally identifiable information from your child’s education record. However, TTU K-12 may disclose appropriately designated “directory information” without written consent, unless you have advised the District in writing. The primary purpose of directory information is to allow TTU K-12 to include this type of information from your child’s education records in certain school publications. Examples include: Honor roll or other recognition lists; Graduation programs; Texas Tech University Publications; News Media and District and University web pages. Directory information, which is information that is generally not considered harmful or an invasion of privacy if released, can also be disclosed to outside organizations without a parent’s prior written consent. Outside organizations include, but are not limited to, companies that manufacture class rings. If you do not wish TTU K-12 is disclose directory information from your child’s educational records without your prior written consent, you must notify the District by checking the appropriate box(es) during the enrollment process. Because of our asynchronous enrollment procedures, this information will be updated annually on April 15th of each year. TTU K-12 has designated the following information as directory information: student’s name, address, photograph, place of birth, publicity from participating in officially recognized activities, major field of study, dates of attendance, grade level, honors, awards received, and the most recent educational agency or institution attended. This action may be reversed at any time by the parent contacting the TTU K-12 Registrar in writing at ttuk12@ttu.edu. In compliance with FERPA, the student’s Social Security number or Student ID is required before releasing any personally identifiable or educational information to the student, counselors or parents.
